Thank you for your interest in visiting Dr. Teachout at Vital Dental Professionals, PC, in Lakeville, NY. We will take the time to diagnose your immediate dental concerns properly, review your medical and dental history, and create a treatment plan tailored to your individual needs and expectations. We will complete a thorough, comprehensive dental evaluation. 


Please download our Patient Forms to get started. Once you have completed all required forms, please bring them to your first appointment. Please do so to avoid having to re-schedule a portion of your appointment, thus delaying any treatment recommended for you.

Scheduling

We will schedule your appointment as promptly as possible. If you have pain or an emergency, every attempt will be made to see you that day. We try our best to stay on schedule to minimize your wait time. Since our dental office provides many dental services, various circumstances may lengthen the time allocated for a procedure. First Visit

Your initial appointment will include a consultation explaining the diagnosis and treatment options. Occasionally, treatment can be done on the same day as the consultation. However, a complex medical history or treatment plan may require an evaluation and a second appointment to provide treatment. Please assist us by providing the following information at the time of your consultation:

  • Any x-rays if applicable (Please have your previous dentist email x-rays in jpeg format to vdp@frontier.com before the visit). In addition to any previous X-rays, we will require a new complete set for those that are not readable.
  • Fill out the required forms.
  • A list of medications you are currently taking.
  • As a courtesy, we will gladly file your dental insurance claim. Please bring your insurance card with you to your first appointment. If your insurance does not reimburse our office directly, we will provide a copy of the treatment needed, and you will be expected to pay for your visit in full on the day of service, and then you will receive reimbursement from your insurance. If you have any questions regarding your insurance or its coverage, please call our office, and we will gladly assist you.

Important: At the consultation visit, all patients younger than 18 must have a parent or legal guardian with them. Then, written consent will be required for each visit.

How To Switch To Dr. Teachout

There are many reasons people look for a new dentist. Their dentist may have retired or moved to a new location, or their dentist may not do the kind of procedures they are looking for. Whatever the reason, if you want to switch to Vital Dental Professionals, PC, here are two steps to make that change. 

1. Transfer your dental records: You must sign a release form from your previous dentist to transfer your records. 


2. Attend your first appointment: You must know what to expect during this appointment to be prepared to provide the necessary information. You should inform Dr. Teachout about any pre-existing conditions and medical treatments you are currently undergoing.


Some medications and conditions require us to check with your physician before completing treatment. The first step Dr. Teachout will take will be to conduct a thorough medical history and necessary dental x-rays. These X-rays will help her diagnose any oral health challenges you’re experiencing and develop the right treatment plan for you.

Home Care Instructions

After extraction:

  • Be sure to follow these instructions carefully after an extraction, as they will help the healing process and prevent later complications such as infection.
  • NO SMOKING, NO RINSING THE MOUTH, NO DRINKING THROUGH A STRAW, NO SPITTING, and avoid hot foods and liquids after extraction that day.
  • Keep gauze packs firmly in position for 1 hour. If oozing persists, replace the packs, and maintain for 1 hour. Apply an ice pack to the outside of your face, next to the extraction areas, on and off for 4-6 hours. Begin saltwater rinses the day after extraction. Place one teaspoon of salt in an 8-ounce glass of warm water. Rinse after meals.
  • If Dr. Teachout prescribes a medication for pain and if an antibiotic is prescribed, be sure to finish the entire prescription as directed.

Care after surgery:

  • Don’t touch! Keep fingers and tongue away from the surgical area.
  • Cool it! Use ice packs on the surgical area (side of the face) for the first 12 hours: apply ice 20 minutes on, 20 minutes off.
  • Still hurts? Take Tylenol every 3 to 4 hours or ibuprofen every 5 to 6 hours for mild discomfort.
  • In pain! For severe pain, please use the prescription medication for pain given to you.
  • Hydrate yourself. Drink plenty of fluids. Do not use straws.
  • Chew Gum. If jaw muscles become stiff, chewing gum at intervals will help relax the muscles. Warm, moist heat on the outside of your face beginning the second day after surgery will help further with muscle relaxation.
  • Eat soft foods! Diets may consist of soft foods, which can be chewed easily and swallowed. Recommended foods and recipes are provided in the menu section. No seeds, nuts, rice, or popcorn!
  • Blood? A certain amount of bleeding is controlled by applying pressure to the surgical area for 90 minutes. Then you may eat or drink. If bleeding persists, a moist tea bag should be placed firmly around bleeding for 1 hour straight.
  • No smoking, please! Do not smoke for at least three days after surgery.
  • Drugs? Discuss this with your doctor or pharmacist to minimize adverse drug interactions if you are on other medications. Do start taking a multivitamin daily if you are not already doing so.

Post-Operative instructions for Root Canal Treatment:

  • If your tooth was abscessed and left to drain, use warm saltwater rinses (1 tsp salt to 8 oz warm water) after every meal.
  • Take the medicines as prescribed by your dentist. Your tooth may feel better after a few days but do not stop taking antibiotics until the recommended time is up. The exception is if you have an allergic reaction to any prescribed medicine. Call our office immediately if this happens.
  • After completion of root canal treatment, a crown or cap is highly recommended.

Post-Operative instructions for Teeth Whitening: 


In the first two day after your your whitening, it is important that you avoid some types of food and drinks to maintain your beautiful results. 

You need:

  • Brush your teeth at least twice everyday
  • Use whitening mouthwash and whitening toothpaste with fluoride
  • Floss at least once a day
  • Avoid tea and coffee
  • Avoid red pasta sauce, soy sauce, BBQ sauce
  • Avoid citric fruit juices 
  • Avoid dark chocolate 
  • A good rule is “if it could stain a white shirt, don’t eat or drink it unil two days have passed” 
  • Discuss with us to create a maintenance schedule for touch ups
